How do you refer an individual?
Referrals are accepted through each person's county Base Service Unit. Prior to receiving services through Laurel Highlands Foundation, Inc., each individual is evaluated by multi-disciplinary teams of professionals.
The following additional information may be requested before an individual can be evaluated:
* Current physical exam
* Mantoux TB test
* DT immunization
* Current psychiatric, psychological & social information
